Resources for Subsidized Child Care Providers

Required Application Information

The following information must be submitted to Provider Support Services:

Licensed Providers

  1. Completed provider enrollment forms
  2. A rate sheet
  3. Daycare policies
  4. A copy of a valid Social Security card or Tax ID number
  5. An INS card if required for validation of SS card
  6. A copy of a valid driver's license or California identification card
  7. A direct deposit application and voided check

Exempt Providers (Alternative Payment Only)

  1. Provider enrollment forms
  2. A copy of a valid driver's license or California identification card
  3. A copy of a valid Social Security card
  4. Proof of residential address (utility bill dated within 30 days)
  5. Proof of negative tuberculosis (TB) test or x-ray from the last six months
  6. A voided check for direct deposit
  7. Birth certificates (aunts, uncles, and grandparents only) to verify relationship to the child(ren)
  8. Exempt from licensing providers must complete the Trustline Application and Fingerprints form
  9. Call 1.800.315.4507 to make a Trustline appointment in advance — our agency code is mailed to the parent

YMCA cannot authorize child care until all documentation is received and a start date is approved. YMCA will not backdate reimbursement for child care services prior to the approved start date.

To enroll as a child care provider with an Alternative Payment Program, please have the client/parent contact their Family Advocate to start the enrollment process. Once the client/family’s Family Advocate has added your name to the enrollment list, a Provider Agreement Packet will be sent to you to fill out and return. Attendance Sheet Responsibilities

  1. Exact time is indicated at the time in for each day of care
  2. Middle section was used only for reporting additional time in and out of care within the same day such as when child attended school; The provider enters the time a school-age child leaves or returns from school during the day.
  3. Exact time is indicated at the time out for each day of care
  4. Any Family Fees owed by and collected from the parent are recorded at the bottom of the Attendance Sheet
  5. Any child absences or variations of approved schedule are explained in the comments column.
  6. Provider indicates invoiced amount/ payment expected for the child care provided.
  7. Provider invoices an amount consistent with their rates on file or the rates charged to non-subsidy families.
  8. Bottom of Attendance Sheet has a full signature and date from client and provider.
  9. Parent and provider use approved YMCA CRS Attendance Sheet; no other attendance record can be used.
